In most cases, yes. Section 1557 of the Affordable Care Act requires covered providers to offer meaningful access to patients with limited English proficiency, and the ADA covers Deaf and hard-of-hearing patients. Providers are expected to use a qualified interpreter rather than a bilingual staff member or a family member.
It's strongly discouraged and, under Section 1557, generally not permitted except in emergencies or when the patient specifically requests it. Relatives often soften a diagnosis, omit details, or lack the vocabulary. A neutral interpreter protects both the patient and the provider.
